There are small white dots on the foreskin

The appearance of white dots on the foreskin may be physiological phenomena such as ectopic sebaceous glands or pearly penile papules. It may also be related to fungal infection, smegma accumulation or inflammatory stimulation. It is recommended to observe whether it is accompanied by redness, swelling, itching and other symptoms. If there is no discomfort, you can leave it alone for the time being. If the symptoms persist, you need to seek medical attention.

Ectopia sebaceum manifests as pinpoint-sized white or light yellow papules, mostly distributed on the inside of the foreskin or the coronal sulcus. It is an abnormal development of sebaceous glands and usually does not require treatment. Pearly penile papules are small white protrusions arranged like pearls, which are common on the edge of the coronal sulcus. They are benign hyperplasia and do not affect health. Both conditions are related to hormone levels or local irritation. Just keep the perineum clean and dry.

If the white dots are accompanied by itching and scaling, it may be caused by Candida albicans infection, which needs to be confirmed by fungal microscopy. People with excessive foreskin are prone to accumulation of smegma due to insufficient cleaning, forming a white granular substance, which may lead to foreskinitis. In rare cases, early stage genital warts or sebaceous cysts may also appear as white protrusions, which need to be identified through acetic acid white test or pathological examination.

Daily care should be taken to clean the inside and outside of the foreskin with warm water every day and avoid using irritating lotions. Choose breathable cotton underwear to avoid prolonged sitting and localized friction. If redness, swelling, exudation or ulcers occur, you should promptly go to a dermatology or urology department and conduct secretion testing or tissue biopsy if necessary. It is not recommended to self-medicate or pick at the affected area to avoid secondary infection or delay in treatment.
